No special preparation is needed, you don't need to fast, it's worth consuming more fluids to make blood collection easier. You should not discontinue chronically taken medications.

Can I take painkillers after platelet-rich plasma injection?

Yes, if it's necessary, but they should not be non-steroidal anti-inflammatory drugs, as they reduce the effect of platelet-rich plasma. You can take: Paracetamol, Pyralgina, Tramal, Doreta.

What does platelet-rich plasma help with?

It helps with degenerative joint changes, menisci, inflammation of ligaments and tendons, bone healing disorders, and partial post-traumatic damage to tendons and ligaments.

How long does it take to see the effects of platelet-rich plasma?

This is very individual, often helps after 3 injections. It's known that the longer the symptoms and inflammation persist, the greater the need for longer plasma administration.

At what intervals is platelet-rich plasma administered?

Depending on need: every 2-6 weeks, best results are achieved by administering plasma every 4 weeks.

Do patients feel pain after platelet-rich plasma injection?

Most patients do not feel pain. In about 10% of patients, pain symptoms may be the same as before plasma injection or stronger and subside after a period of several hours to several days. Very rarely do they last longer.

What to do in case of pain or swelling after platelet-rich plasma injection?

You should apply ice to the injection site, possibly take painkillers, e.g., Paracetamol, Pyralgina. In case of persistent symptoms, you should contact the doctor through the Medical Center.

My professional DNA was shaped by hundreds of shifts and thousands of consultations at the Clinic of Traumatology and Orthopedics of the Military Institute of Medicine. I deal with the full spectrum of injuries: from multi-site injuries to complicated joint fractures, hip socket reconstructions, or pelvic fractures. I also treat overuse and degenerative conditions of the shoulder, elbow, wrist, hip, knee, and foot, always starting with what is most beneficial for the patient: conservative therapy tailored to the goal or a precisely planned procedure.

In surgical work, I combine planning based on modern diagnostics and 3D imaging with modern angular stability plate fixations and reconstructive techniques that restore the anatomical geometry of joints. In conservative treatment, I use methods that support tissue regeneration (including platelet-rich plasma, hyaluronic acid, Goldic, Orthokine, stem cells - SVF), so that the patient returns not only to "daily life," but to the activity they love. My priority is always pain-free movement and return to functionality.

Science and teaching are a natural extension of my clinical practice for me. I defended my doctoral thesis on the results of surgical treatment of distal humerus fractures, I am a member of specialized orthopedic organizations, including Faculty AO Poland, Key Opinion Leader Stryker, I conduct training and share experience with younger doctors.

PRP Therapy

Joint Treatment with Platelet-Rich Plasma PRP

Joint pain, limited mobility, and recurring inflammation are problems that can significantly impair daily functioning. These ailments can affect both physically active individuals and patients with degenerative joint disease, after injuries or overloads.

One method that has gained significant importance in orthopedics and regenerative medicine in recent years is Platelet-Rich Plasma (PRP) therapy, which involves treating joints with the patient's own biological material.

Platelet-rich plasma PRP is a concentrate of platelets obtained from the patient's blood. The procedure begins with drawing a small amount of venous blood, similar to a standard laboratory test. The blood is then centrifuged in a special device to separate the plasma fraction with a high concentration of platelets.

Why is this important? Platelets contain growth factors and biologically active substances that support tissue repair processes, reduce inflammatory responses, and improve the intra-articular environment.

PRP is not a medication that "masks" pain, but a method that works at the root of the problem, stimulating the body toward self-regeneration.

Most common indications for PRP therapy:

Degenerative joint disease (e.g., knees, hips, shoulders, elbows, wrists, ankle joints)
Chronic pain and overload of joints, tendons, ligaments, and muscles
Post-sports injury conditions
Cartilage damage
Inflammation and irritation of periarticular structures
Complaints limiting physical activity despite rehabilitation
Limited joint mobility related to multi-tissue contracture

PRP is often part of combined treatment, combined with physiotherapy, appropriately selected movement, load reduction, and orthopedic diagnostics.

The effects of platelet-rich plasma treatment are individual, as they depend on the type of problem, the degree of joint changes, the patient's age, and activity level. Most commonly observed are:

Gradual pain reduction
Improved mobility and comfort of movement
Reduction of inflammatory conditions
Improved exercise tolerance
Support for tissue regeneration
Improved range of motion in joints

It is worth noting that PRP typically works progressively, meaning improvement may appear gradually in the weeks following the procedure, as well as after subsequent doses of plasma, which multiply the therapy's effect.

Before planning PRP treatment, it is necessary to determine the cause of the complaints:

Which joint is the source of pain
Whether the problem concerns articular surfaces, tendons, ligaments, menisci, or labral structures
Whether there are degenerative changes, inflammation, or overload
Whether the patient has contraindications to therapy (very rare)

In most cases, an important element confirming the diagnosis is imaging studies: X-ray, ultrasound, or MRI.

The outpatient procedure is short and well-tolerated:

Drawing blood from the patient
Centrifuging the sample and preparing PRP
Administering the plasma to the joint under appropriate aseptic conditions
Post-procedure recommendations regarding joint protection, activity, and possible rehabilitation

Some patients may experience temporary discomfort in the joint area, a feeling of fullness, or increased pain for 1-3 days. This is usually the body's response to regenerative processes.

In clinical practice, the best results come from treatment that doesn't end with just administering the preparation. PRP can work more effectively if the patient:

Reduces overloads (e.g., training or occupational)
Implements physiotherapy tailored to the problem
Takes care of stabilizing muscles around the joint
Applies a gradual return to activity

The doctor may also recommend temporary limitation of intense exercise so the joint has conditions for regeneration.

When treating joints with platelet-rich plasma, it's easy to focus solely on the preparation itself, the "valuable biological material" that enters the joint. This is obviously very important, but in clinical practice, two other elements are equally important: precision of administration and the doctor's experience.

PRP administration should be performed in the right place, in the proper layer, taking into account the anatomy of the specific joint and the cause of pain. It is the procedure technique, proper qualification, and years of practice that affect the safety and real effect of therapy.

I personally pay special attention to this because I know that PRP produces the best results when the procedure is backed by precision of execution and years of experience from a doctor who adapts the administration technique to the patient's degeneratively altered anatomical conditions.

Case studies - treatment results

Proximal humerus fracture

A 76-year-old female patient suffered a fracture from a fall from standing height. The fracture was multi-fragmentary with significant angular displacement of fragments. Without surgical treatment, the patient would not have regained good range of motion and muscle strength.

Surgery was performed involving open reduction of the fracture with proper alignment of bone fragments and fixation using a plate with angular stable screws. Additionally, reconstruction of the damaged shoulder ligament apparatus was performed and postoperative rehabilitation was initiated.

After 4 months, the patient regained normal range of motion, muscle strength, and full limb function.

Distal humerus fracture

A 74-year-old female patient suffered an elbow injury from a fall on a slippery surface. She sustained a multi-fragmentary fracture of the distal humerus with significant comminution of the articular surface.

The greatest problem was the comminution of the articular surface. The elbow joint has very closely matched articular surfaces, so any irregularity in the joint causes significant limitation of range of motion.

Surgical treatment was performed, involving reduction of the articular surface of all small articular fragments and fixation of the fracture with plates (and Kirschner wires). Additionally, the fracture was accompanied by a radial head fracture, which was fixed with cannulated screws.

After 6 months, the patient regained full functional range of motion (extension limitation remained, but did not affect limb function).

Radial head fracture

A 48-year-old male patient suffered a multi-fragmentary fracture of the radial head and neck from a fall on stairs.

This type of fracture causes significant limitation of range of motion, especially rotational movement. This causes great difficulties in performing daily activities.

Surgical treatment involved open reduction of the fracture and fixation with a plate with angular stable screws.

After 3 months, the patient regained functional range of motion of the joint.

Distal radius and ulna fractures

A 28-year-old male patient suffered multi-organ trauma (head injury, chest injury, unconscious for 14 days) and an open multi-fragmentary fracture of the radius and ulna from a traffic accident.

Initially treated at another hospital, where after regaining consciousness, amputation of the hand was proposed due to a crush injury of the limb, unsuitable for reconstruction. The patient did not consent, and in an appeal process, he was transferred to WIM Hospital.

The patient was informed that an attempt at reconstruction would be made, but the outcome was uncertain. During a 4-hour operation, reconstruction of the broken bones was performed with bone graft supplementation of bone defects and fracture fixation with plates.

The surgery and long-term rehabilitation allowed for a good treatment outcome.

Distal femur fracture

A 22-year-old female patient, after a motorcycle traffic accident, was surgically treated outside WIM Clinic for a multi-fragmentary comminuted open fracture of the distal right femur.

One year after treatment, destabilization of the fixation occurred. Re-reduction of the fracture was performed, correction of axis, rotation and length of the femur, and fixation with two plates with angular stability.

Bone healing proceeded slowly due to the initial significant bone defects. In the further course of treatment at WIM, treatment stimulating bone healing was used, including bone grafts, bone marrow grafts, and platelet-rich plasma was administered multiple times.

Bone union was achieved, and the patient returned to daily activities.

Proximal tibia fracture

A 39-year-old female patient suffered a severe knee joint injury during windsurfing. Transported by ambulance to WIM.

Surgical procedure was performed for reduction and fixation of all columns of the proximal tibia with fixation using four plates with angular stable screws, suturing of both menisci, reconstruction of the lateral collateral ligament and medial collateral ligament.

Long-term rehabilitation allowed for return to full limb function and enabled return to sports.

Distal tibia and fibula fracture

A 42-year-old male patient suffered an ankle injury from a fall from a height of 2 meters. At WIM, a PILON type fracture was diagnosed and qualified for surgical treatment.

Due to the comminuted nature of the fracture, surgical reduction of the fracture with multi-plate stabilization was performed.

In the postoperative period, intensive rehabilitation allowed return to full function after 6 months. After another two months, the patient reported ankle pain.

Through diagnostic imaging, secondary displacement of fracture fragments was found, caused by excessive use of the limb. Another operation was performed with re-reduction of the fracture fragments.

The patient returned to full function after another 6 months.

Post-traumatic lateral meniscus injury

A 40-year-old female patient, operated outside WIM for an old meniscus injury from a skiing fall (12 years after the fall), because pain symptoms intensified during a squash game.

At that time, a meniscus repair procedure and removal of necrotic tissue from inside the knee joint were performed. The patient was informed about the need for further surgical procedures, including cartilage transplantation.

After consultation at Ortogo, a non-surgical treatment plan was established using platelet-rich plasma and hyaluronic acid.

After several intra-articular injections, significant pain relief was achieved. After 8 months, the injection procedure was repeated, and after another 3 months, complete pain relief and return to full function and sports were achieved.

Overload sacroiliac joint inflammation

A 42-year-old female patient has been reporting severe sacroiliac joint pain for several years, both spontaneous and post-exertional.

The clinical picture was unclear because it was accompanied by sciatic nerve inflammation. Treated pharmacologically, which resolved the sciatic nerve inflammation symptoms.

Clinical examination revealed marked tenderness over the sacroiliac joint. Additional diagnostics (magnetic resonance imaging) showed features of sacroiliac joint inflammation.

Qualified for treatment with platelet-rich plasma. After a series of intra-articular and periarticular injections, symptoms resolved, and the patient returned to activity.

Degenerative changes of the knee joint

A 68-year-old female patient with degenerative changes of the knee joint with a long history of pain syndrome.

Examination revealed pain on the inner side of the knee (medial compartment), limitation of range of motion, including a 20-degree flexion contracture (lack of full extension in the knee). Additional tests (ultrasound, X-ray) confirmed the diagnosis of degenerative changes.

The patient wanted to avoid or delay the need for surgical treatment. Non-surgical treatment was planned using platelet-rich plasma and hyaluronic acid.

A series of intra-articular injections were performed to regenerate articular surfaces and menisci. Additionally, periarticular injections were performed to eliminate inflammation of periarticular ligaments and capsular-flexion contractures.

As a result of the treatment, pain decreased to such an extent that it no longer interferes with daily functioning. Additionally, periarticular contractures resolved.

Jumper's knee

A 37-year-old female patient presented with chronic knee pain, especially when walking up stairs, attempting to kneel, squat, and when getting up from a chair.

Symptoms also worsened when the patient maintained knee flexion for an extended period. The patient had previously been operated on multiple times outside WIM (11 knee arthroscopies).

Clinical examination revealed mild patellofemoral joint pain and severe pain at the patellar tendon attachment to the patella. Additionally, mild soft tissue swelling in the area of the lower pole of the patella was found.

Non-surgical treatment was planned using platelet-rich plasma. Platelet-rich plasma preparation was administered multiple times at the patellar tendon attachment site.

Pain resolved, and the patient returned to full function after just 3 months.

Painful shoulder syndrome

A 43-year-old male patient with painful shoulder syndrome with pain symptoms for 9 months, after a shoulder injury during sports activity.

Magnetic resonance imaging revealed partial damage to the tendons of the muscles forming the rotator cuff. Additionally, subacromial bursitis was found on MRI.

Symptoms significantly limited shoulder mobility and worsened during sleep. The patient also reported neck pain.

Non-surgical treatment was planned with platelet-rich plasma administration. Several series of injections were performed, intensive rehabilitation was included, and symptoms resolved with return to full mobility.

A fantastic specialist who saved my leg after a talus fracture (into several pieces) and ligament rupture. I underwent a very difficult operation, in my opinion, after which I was left with 6 permanent screws. 4 in the talus, 2 in the tibia.
